Certificate of Completion Duplication Fee
If your Certificate of Completion (Page 7 of the L.V.M.P.D. CFP application) physically issued at the end of your course, is lost or destroyed we will need to create a new physical copy and mail it to you. It cannot be faxed or emailed, as you will need to submit the original handwritten copy signed and initialed by you and your Instructor.
